- Click below on “Course Survey” to access the survey. After completing the survey, return to the course page.
- After you submit the evaluation, click Mark Complete and you will be able to download or print your certificate of completion.
- If you are a physician requiring a CME certificate, copy or make note of the code you receive during the evaluation. On the next page you will follow a link to enter that code and receive your CME certificate.
