Providing safer, effective pain care has been a topic of significant debate as society navigates the current opioid misuse crisis. The legacy of aggressive opioid prescribing continues to affect both patients and providers even as opioid prescribing is at a 30 year low. The attempt to provide guidance surrounding opioid prescribing, via the 2016 CDC Guidelines, created unforeseen consequences. The patient’s voice was often eliminated, and the guideline misapplication led to forced tapers, inadequate acute pain control, and worsening mental health, including suicidality. This resulted in a multi-year revision based on patient, provider, and other stakeholder’s input. The 2022 CDC guidelines represent a significant refinement of 2016, are expanded to include all patient care settings, and prescribing guidance for both acute and chronic scenarios. They provide support in determining treatment, agent selection, duration, and assessing risk. This presentation will review each of the guidelines, the ‘why,’ and their potential impact. Shared decision making, individual patient risk assessment, and goal- directed care will also be reviewed. While a paucity of tools exists to fully operationalize risk benefit assessment, recommendations will be provided for providers, patients, and policy makers. Effective application, for both professional organizations and regulatory agencies, has the potential to improve quality of life and reduce burn-out for patients and their caregivers.
**In addition, research has shown that in the past five years, patients who are on long term opioids for chronic pain simply do not migrate towards fentanyl or other non-prescribed opioid use.

  3. Reduce stigma surrounding patients who require opioids for function.
  4. Explore how the guidelines formal adoption can support a variety of state-wide professional organizations policy implications.
