Online Course Evaluation: Instructions

After the live meeting ends:

1. Log into this online course
2. Click below on “Course Survey” to access the survey. It will open in a new tab – be sure to leave this tab open!
3. After you submit the evaluation, return to this tab and click Mark Complete. If you encounter an error message about your certificate, please refresh the page. 
4. You will now be able to download your certificate. Please be sure to select the correct one! There is one for physicians (MD and DO only) and one for all other participants.
