This session explores the overlapping presentations of Intoxication, Delirium, and Psychosis in the clinical context of Substance Use Disorders.  We will review the definitions of each of these Psychiatric diagnoses and clinical presentations.  We will use some brief discussion cases to identify the differential thinking needed to diagnose and treat these complicated patients.

Learning Objectives:
Review the criteria for diagnosing intoxication, delirium, and psychosis
Differentiate these diagnoses when presenting separately and overlapping in SUD patients
Classify psychotropic drugs used to treat these conditions
Review the neurotransmitter pathways involved in the conditions and the indications for treatment.

Presenter

William Allen Schaffer, MD Dr. Allen Schaffer is a Physician practicing in New England. His interests include Collaborative Care Psychiatry, Consultation Liaison Psychiatry, Psychiatric Inpatient Service, Co-occurring Substance Use Disorders, and Treatment of Impaired Professionals. He has a unique set of clinical and administrative skills bridging many of the delivery sectors in health care. He is ABMS Certified in both Psychiatry and Internal Medicine, and he practices Psychiatry across the care continuum. He is currently an Attending Inpatient Psychiatrist at the University of Vermont Medical Center in a long term Locums assignment.
He previously practiced in Bangor, Maine, concurrently as Chief of Psychiatry at Community Health and Counseling Service, Attending Psychiatrist at St. Joseph Hospital, at Wellspring Substance Abuse and Mental Health Service, Aroostook Mental Maine Board of Licensure in Medicine for nearly a decade.
Dr. Schaffer resumed full-time clinical practice in 2006 following 20 years as a Medical Executive in three Fortune 50 healthcare companies. He served from 1993 to 2005 as Chief Medical Officer for CIGNA. Prior to that he was head of professional affairs and quality management at Aetna; and led Hospital Quality management and Primary Care delivery programs at Humana. As part of these duties he held faculty appointments with regular patient care duties in the Department of Internal Medicine at the University of Louisville, and University of Connecticut Schools of Medicine; and later in the Department of Psychiatry at the University of Vermont School of Medicine. He has also administered not-for-profit hospital programs, and founded and managed a multi-office medical group.
Dr. Schaffer received his medical degree in 1975 from the University of Washington, Seattle, where he was granted early admission, received medical thesis honors and the Robert H. Williams Medical Research Award. He obtained Residency training at the Tulane Service of Charity Hospital in New Orleans and at Baptist Memorial Hospital in Memphis. In 2005 he chose to pursue board certification in Psychiatry and completed his Residency in Psychiatry at the University of Vermont in 2009.
Dr. Schaffer’s past trustee appointments include Bazelon Center for Mental Health Law, the National Committee for Quality Assurance (NCQA), the American Association of Health Plans (AHIP), and the National Advisory Board for the Agency for Healthcare Research and Quality (AHRQ). He has published original articles in the New England Journal of Medicine and The Annals of Internal Medicine. Health Center, and 3 Regional County Jails.
